Qualified Mental Health Professional (QMHP)

 

Job Title: QMHP

 

Immediate Supervisors: Program Director, Filed Supervisor, Clinical Supervisor

 

Qualifications:  QMHP will have at least a bachelor's degree in human services or related field from an accredited college in, social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling, or other degree deemed equivalent to those approved via DMAS for QMHP.  QMHP will have at least 1-2 years of case management experience and/or counseling experience working with individuals (1) with cognitive deficits (2) with physical disabilities (3) with other mental health issues that may require support counseling. The QMHP will be trained in the following areas: First Aid/CPR, HIV/Bloodborne Pathogens, Human Rights, and Ethics. QMHP will have at least one year of documented experience providing direct services (developing, conducting, and approving assessments and individual service plans) with individuals who have an intellectual disability (mental retardation) or other developmental disabilities. QMHP must be certified by the Virginia Department of Health Professions Board of Counseling.

 

Position Description/Responsibilities: The QMHP will provide mental health support services to individuals 21 years and older. The QMHP will be responsible for developing and implementing treatment plans in accordance with the program description and the client’s needs.  QMHP will develop a written Individual Service Plan after a clinical assessments and diagnoses for the purpose of establishing treatment goals and objectives are created. QMHP is also required to complete Quarterly Reports and ISP Reviews to track the client’s progress while receiving services. QMHP will be responsible for recommending appropriate services for the client and aid the client in collaborating with these services. QMHP will obtain bi-weekly contact with the referral agency (if applicable) and will conduct appropriate discharge planning when necessary. QMHP will be responsible for planning, implementing, and evaluating treatment plans using treatment interventions to facilitate human development and to identify and remediate mental, emotional, or behavioral disorders. QMHP will be required to complete weekly progress notes to document the client’s weekly progress as it pertains to the client’s goals. QMHP  will also be required to attend individual and group supervision bi-weekly to receive clinical feedback on the client’s progress while receiving MHSS.

Certified Substance Abuse Counselor  (CSAC)

 

IMMEDIATE SUPERVISOR: Program Director/Clinical Director

 

MINIMUM QUALIFICATIONS: 12VAC35-105-410

 

Education and Experience:

• Bachelor's Degree in Counseling or Healthcare Related Field required.

• Experience with Addiction and Behavioral Heath patients required.

• Licensed as a Certified Substance Abuse Counselor(CSAC), CSAC-A or CSAC-S by VADHP required.

• First Aid/CPR, HIV/Blood borne Pathogens, Human Rights and Ethics.

 

Responsibilities:

 • Assists clients/patients in identifying self-determined goals and jointly prepares individualized service plans.

 • Provides consultation, education and prevention identification services to clients within the residential setting.

• At least 1-2 years of case management experience and/or counseling experience working with individuals (1) with cognitive deficits (2) physical disabilities (3) other mental health issues that may require support counseling.

 • Interventions made appropriately during patient crisis, i.e. AMA, depression, crisis issue, etc. in order to maintain the patient in the community for the purpose of working on his/her addiction and keeps staff/supervisor informed.

 • Discharge summaries for inpatients and outpatients are completed and reviewed with the client before the time of discharge and include all elements per department, State and TJC guidelines.

• Documents and charts information about client assessment, diagnostic studies, individualized service plans correspondence, medical records, etc., prepares and submits records as requested

.• Monitors and audits clinical and medical records and practices for compliance with regulatory, reimbursement and agency standards; reviews documentation for accuracy; assists with reviewing division policies and procedures.

• Performs intake assessments, client assessments, and diagnostic interviews; conducts mental health and substance use disorder assessments, and monitors level of functioning appropriately.

 • Conducts clinical assessments, formulates individualized treatment plans based on diagnoses and problems identified by the patient and the treatment team, completes all designated documentation and service authorization documentation, and formulates follow-up and discharge planning based on identified needs.

 • Provides case management services; coordinates and links clients to community resources and medical services, interfaces with other service providers and the clients’ support systems.

 • Managed Care companies are contacted in timely fashion for case reviews. Verifies insurance coverage and documents benefits
